Job summary
UnitingCare Children, Young People and Families in Caloundra is seeking a Supported Living Facilitator to provide in-home personal care and daily living assistance to clients in the community. The role involves visiting clients, supporting independence and coordinating with their Care Plans.
You will drive between client homes using your own vehicle, with hours mainly Monday to Friday, 9am–5pm. CPR/First Aid and a valid driver’s licence are essential; experience in disability, aged or community
Qualifications
- Current CPR and First Aid Certificate is required before you start.
- Open Driver's Licence and access to a vehicle is needed.
- Ability to communicate effectively in written and spoken English.
- Experience in Disability, Aged or Community sectors is beneficial but not essential.
Responsibilities
- Provide in-home personal care and daily living assistance (showering, meals, housekeeping).
- Travel between clients using your own vehicle.
- Work mainly Monday to Friday, 9am–5pm.
- Support clients per their individual Care Plan and assist with community activities.
- Transport and assist with shopping and appointments.
