Zusammenfassung

Essential Nursing and Disability Services is seeking a Clinical Team Leader to oversee clinical leadership and the day-to-day operations of SIL houses and related supports across the North and South of the river in Western Australia.

You will lead a team of Support Workers, ensure clinically competent care aligned with participant goals, manage medication safety, and drive compliance with NDIS Practice Standards and WHS policies.

Qualifikationen

• Current AHPRA Registered Nurse.
• Minimum 5 years of paid RN experience.
• 2–3 years of leadership/management in clinical, disability, community care or similar setting.
• Current First Aid & CPR.
• National Police Clearance.
• NDIS Worker Screening (or ability to obtain).
• Valid WA Driver's Licence and reliable vehicle.
• Evidence of required vaccinations.

Aufgaben

• Provide clinical oversight, assessments and escalation of participant health needs.
• Lead safe medication management, MARs, storage and documentation.
• Lead infection control practices and clinical risk management.
• Ensure care plans and clinical documentation are current and accurate.
• Lead daily SIL house operations and ensure smooth service delivery.
• Manage rostering, shift coverage and operational workflows.
• Supervise, coach and mentor Support Workers.
• Support onboarding and competency development.
• Address performance issues and escalate as needed.
• Promote a respectful, values-driven team culture.
• Ensure WHS compliance, incident reporting and corrective actions.
• Coordinate communications with participants, families and health partners.
