Job summary

Seed Employment in Bundaberg is seeking a Community Support Worker to strengthen local First Nations communities. You will provide practical support, advocacy and holistic case management, helping clients access health, housing, employment and family services.

You will build genuine relationships, maintain client records, and collaborate with community organisations and government services to deliver inclusive programs that promote wellbeing and social participation.

Qualifications

• Build genuine relationships with clients, community members and service providers.
• Strong communication and interpersonal skills.
• Confidence working with clients requiring practical support and referrals.
• Good administrative and record-keeping skills.
• Ability to work collaboratively across community and government services.

Responsibilities

• Engage community members in social health and wellbeing programs.
• Provide holistic case management and maintain accurate client records.
• Link clients with services including health, housing, employment, family support, childcare, family violence, social support and alcohol and other drug services.
• Develop and maintain relationships with community organisations and other service providers.
• Support the development and delivery of community programs and projects.
• Promote social inclusion and participation for vulnerable and disadvantaged clients.
• Liaise with government and community services to support effective service delivery.
• Support activities that encourage physical activity, participation and wellbeing.
