Job summary

Seed Employment is seeking a Family Support Worker to provide culturally responsive early intervention, case management and practical support to First Nations families across Bundaberg, North and South Burnett and the Fraser Coast. Regular travel within the service region is involved.

You will collaborate with families, community services and government agencies to connect clients to services, build capacity and support safe home environments.

Qualifications

• Minimum 12 months' current relevant experience, or approximately two years of extensive relevant experience.
• Commitment to culturally appropriate support for First Nations individuals, families and communities.
• Good communication, interpersonal and time-management skills.
• Current driver's licence.
• Senior First Aid Certificate, Blue Card and Federal Police Check are required; however, these can be completed upon employment.
• Certificate III or IV in Aboriginal and Torres Strait Islander Primary Health Care, or current study towards this qualification, is desirable.

Responsibilities

• Providing holistic case management and needs assessments
• Delivering early intervention and family support strategies
• Supporting families with parenting skills, safe environments and building family capacity
• Linking clients with appropriate community, health and wellbeing services
• Working collaboratively with Child Safety Service Centres and Regional Intake Services
• Building strong professional relationships with relevant agencies and stakeholders
• Maintaining clear, accurate and confidential client records
• Supporting clients with referrals, education, practical assistance and approved transport
• Participating in case management, supervision, training and debriefing sessions.
