Zusammenfassung

NSW Health, within the Sydney Children’s Hospitals Network at Westmead, is seeking a Priority Populations Care Navigator to advance the PEACH program and improve access for children and young people from priority populations.

You will collaborate across SCHN, manage project tasks, monitor KPIs, and drive change with staff, families and carers to ensure sustainable impact beyond funding.

Qualifikationen

• Tertiary qualification in a health care discipline or equivalent experience.
• Demonstrated experience in project management, change management and quality improvement in a health setting.
• Highly developed interpersonal and communication skills with strong written and presentation abilities.
• Self-directed and able to prioritise work in an environment with competing priorities.
• Understanding of how diverse backgrounds can impact access to health care systems.
• High level influencing, negotiation and facilitation skills to drive organisational change.
• Proficient computer literacy and IT skills to support quality improvement and project management initiatives.

Aufgaben

• Coordinate PPCNs across SCHN to implement the PEACH pathways.
• Monitor KPIs and report progress to governance groups.
• Drive cultural change through staff engagement with families and carers.
• Collaborate with Local Health Districts and SCHN departments and treating teams.
