St Vincent's Health Australia in Hawthorn East is seeking an Enrolled Nurse to join a warm, patient‑focused team at St Vincent’s Care Services Hawthorn (Mary Mackillop Aged Care). You will provide quality, person‑centred nursing care and collaborate with RNs and the care team to support residents’ wellbeing.

Qualifications

• Current registration with AHPRA as an Enrolled Nurse.
• Diploma of Nursing or equivalent.
• Knowledge of the Aged Care Quality Standards.
• NDIS Worker Screening Check may be a requirement of this role.

Responsibilities

• Delivers person centred care that coordinates individualised care from admission and throughout their stay, incorporating planning of care in collaboration with Registered Nurse.
• Provision of detailed documentation of a systematic comprehensive and accurate assessment of residents.
• Coordinates and implements the intervention(s) identified in the plan of care/clinical path.
• Evaluates and records progress towards expected outcomes and revises the care plan as necessary.
• Communicates resident response to treatment and care and documents accordingly.
• Facilitates communication between the resident, family, medical, nursing, ancillary and allied health staff.
• Participate in clinical risk management, resident safety and continuous quality improvement activities as part of day-today work.
• Acts as a preceptor for new Personal Care staff.
