Job summary

Optimum Health Solutions is seeking an experienced Behaviour Support Practitioner to join our multidisciplinary team across NSW clinics, delivering evidence-based Positive Behaviour Support and developing Behaviour Support Plans.

You will conduct assessments, create Plans, and support clients in clinic, home, community and school settings, working with children, adults and families.

NDIS registration required; we value growth, professional development and collaborative practice.

Qualifications

• Current NDIS Behaviour Support Practitioner registration (Core level or above).
• Experience as a Behaviour Support Practitioner or in a similar behavioural support role.
• Strong understanding of Positive Behaviour Support frameworks.
• Experience conducting assessments and developing Behaviour Support Plans.
• Excellent communication and relationship-building skills.
• Ability to work independently while contributing to a multidisciplinary team.
• A calm, patient and solution-focused approach.
• A genuine passion for supporting people with disability and their families.

Responsibilities

• Conduct behavioural support assessments.
• Develop high-quality Behaviour Support Plans.
• Deliver evidence-based Positive Behaviour Support interventions.
• Provide support in clinic, home, community and school environments.
• Work with children, adolescents and adults with complex support needs.
• Collaborate with families, carers, support coordinators and other stakeholders.
• Contribute to meaningful outcomes that improve quality of life and community participation.
