Responsibilities

• Provide personalised support with daily living activities including personal care, hygiene, meal prep and household tasks.
• Facilitate community participation by accompanying clients to social activities and appointments.
• Support clients in developing life skills such as money management and self-advocacy.
• Maintain accurate records of support provided and monitor changes in client needs.
• Communicate effectively with clients, families and team members to ensure coordinated care.
