Job summary

Healthscope Independence Services (HIS) is seeking a passionate Disability Support Worker to deliver high-quality care within our Supported Independent Living (SIL) services under the NDIS.

You will support daily living, personal care, and community activities, document progress, and collaborate with a multidisciplinary team. Based in Melbourne, Victoria, Australia, the role offers SCHADS-based pay, learning and development, EAP, and paid training time.

Qualifications

• Certificate IV in Disability or related qualification.
• Minimum of 1 year experience in disability/healthcare.
• Strong communication and organisational skills.

Responsibilities

• Provide personal care support in line with individual support plans.
• Assist with daily living activities including meal prep and feeding.
• Help with household tasks (cleaning, laundry, etc).
• Support clients in community activities, appointments and recreation.
• Ensure documentation (daily notes, progress reports) is up to date.
