Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator (Disability Services, NDIS) in Worongary and Parkwood on a casual basis. You’ll work with the person supported, families and networks to help achieve life goals.

Responsibilities include assisting with daily living, personal care, developing and reviewing individual plans, promoting autonomy and rights, privacy and dignity, with flexible hours and English communication.

Qualifications

• Experience in disability or aged care is preferred but not essential.
• Current CPR and First Aid Certification required before start.
• NDIS Worker Screening check required prior to employment and assistance available to apply.

Responsibilities

• Work in partnership with the person being supported, their family and informal support networks, to attain hopes and aspirations they have established for their lives
• Provide support to people in everyday living situations, including personal care as identified, with an emphasis on choice and quality of life
• Participate in the development, implementation and review of individual plans that focus on life hopes and aspirations
• Enable people to attain personal autonomy over their own lives and the services they receive
• Uphold the right to respect, privacy, dignity and confidentiality of the person being supported and their family
