Job summary

Justice Health and Forensic Mental Health Network is seeking an Aboriginal Health Practitioner, Primary Care, in Broken Hill. The role focuses on culturally safe care within a multidisciplinary team and supports patients transitioning from custody back into the community.

The successful candidate will meet AHPRA registration for the Practitioner track or hold necessary Aboriginal Health Worker qualifications and be prepared for travel across NSW.

Qualifications

• Aboriginal Health Practitioner must hold Certificate IV and be registered with AHPRA
• Aboriginal Health Worker must hold or aspire to hold Certificate III; transition to Aboriginal Health Practitioner upon completion and registration with AHPRA
• Eligible to drive in NSW and travel as required

Responsibilities

• Provide flexible, holistic, and culturally sensitive primary health care to Aboriginal and/or Torres Strait Islander people
• Focus on prevention and management of chronic disease for Aboriginal clients
• Collaborate with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations and other groups to ensure timely care
