Job summary

Northcott is seeking a Coordinator to support families through a child- and family-centered approach, providing evidence-based interventions for children with developmental delays or disabilities. You will use higher qualifications in Early Childhood, Allied Health, or Social Work to guide decisions and referrals.

The role involves completing functional assessments, connecting families to services, and guiding NDIS plan reassessments to improve outcomes.

Qualifications

• Use mydiploma or higher qualifications in Early Childhood, Allied Health, Social Work, or a related field to support evidence-based decision-making.
• Experience working with families and children in developmental contexts is preferred.

Responsibilities

• Support families through a child and family-centred approach, ensuring children receive the appropriate interventions based on best practice evidence.
• Complete functional assessments for children with developmental concerns or disabilities, determining their needs and ensuring timely referrals to appropriate support pathways.
• Connect families to community services and mainstream programs to enhance their social participation and access to essential services.
• Guide families through NDIS plan reassessments, gathering evidence, assisting with goal development, and ensuring the right support is in place.
• Utilise the NDIS plan to support children and their families with improved choice, control, social participation, and overall quality of life.

The NDIS Partners in the Community Program (PITC) supports the National Disability Insurance Scheme (NDIS) to be implemented locally.

As an NDIA partner, Northcott will deliver the NDIS Early Childhood Approach (ECA) service to local communities in Northern NSW, Mid North Coast, Hunter New England, North Sydney, and Western Sydney. The EC approach supports activities that improve the independence and social participation of children aged 0-9 with developmental delay and/or disability.
