Job summary

Disability Aged Community Care Services (DACCS) is a Logan area provider delivering personalised support to individuals with disabilities in their homes and communities. This role supports daily living activities with compassion and respect for client autonomy.

Applicants should hold a Certificate 3 in Individual Support, NDSE screening, blue card and a valid Queensland licence; experience with diverse clients is valued and ongoing training is provided.

Qualifications

• Certificate 3 in Individual Support (Aging, Disability, Home and Community) required.
• NDIS Worker Screening Card and Blue Card required.
• Current Queensland Driver's Licence.
• Experience providing personalised support to individuals with disabilities.
• Excellent communication, rapport with clients, families and healthcare professionals.
• Empathy, patience and genuine passion for helping others.

Responsibilities

• Delivery of personalised support services to clients with high-quality care and compassion.
• Assist clients with daily living activities: personal care, meal prep, light housekeeping.
• Work collaboratively as a team to maintain safe, consistent and high-quality support.
• Advocate for client rights and ensure access to resources and services.
• Maintain detailed records and documentation to track client progress and support service delivery.
• Participate in ongoing training and professional development to enhance skills.
