Vision and Care is a registered NDIS service provider based in Adelaide. We are seeking qualified Disability Support Workers to deliver person-centred support to participants with diverse needs, including high-risk behaviours.

This role requires compassion, safety focus, and the ability to implement support plans in real-world settings. You will work across Supported Independent Living (SIL), community settings and in-home care, coordinating with families and health professionals to promote

Formación

• Minimum qualification: Certificate IV or Diploma in Disability, Community Services, Individual Support, Mental Health or another closely related field.
• Minimum experience: At least two (2) years of full-time work experience in disability support services or a closely related field.
• High Behaviour Support Experience: demonstrated practical experience supporting participants with complex needs and challenging behaviours.
• Proven ability to apply positive behaviour support strategies, behaviour management techniques and de-escalation.
• Experience with NDIS participants in SIL, group homes, community care or similar settings.
• Strong understanding of the NDIS framework, rights, duty of care and WHS.
• Ability to work independently and in a multidisciplinary team; strong communication and documentation skills.
• NDIS Worker Screening clearance and other employment checks; Current First Aid/CPR certification or willingness to obtain.

Responsabilidades

• Provide individualized care and support to NDIS participants with diverse disabilities.
• Support participants with complex and high-risk behaviours, including de-escalation.
• Implement and follow individual support plans and Positive Behaviour Support Plans.
• Identify triggers and respond safely to challenging situations.
• Assist with personal care, hygiene, dressing and daily living activities.
• Support meal preparation, household tasks and medication assistance where authorised.
• Provide support across SIL, group homes, community settings and private residences.
• Maintain progress notes, behaviour monitoring records and incident reports.
• Support participants to develop independence and daily living skills.
• Coordinate with families, coordinators, allied health and other team members.
