Job summary

CareChoice is seeking passionate Disability Support Workers to provide high-quality, person-centred care to people with complex support needs in Victoria, Australia.

Responsibilities include enabling participants to reach their goals across settings such as supported living, community access, transport, and domestic/personal care. Essential requirements include a relevant Certificate III/IV, First Aid, NDIS Worker Screening, and a valid Victorian driver’s license.

Qualifications

• Experience delivering person-centred care to people with complex needs.
• Excellent communication and problem-solving abilities.
• Flexible and reliable with team collaboration.
• Ability to work independently and as part of a team.
• Enteral feeding (PEG) experience is desirable.

Responsibilities

• Provide support across settings including complex 24-hour supports.
• Assist participants to reach their goals and improve independence.
• Support in - living arrangements, community access, transport, and personal care.
• Deliver domestic and personal care in participants' homes and settings.
• Maintain participant dignity, safety, and wellbeing at all times.

Essential Requirements
