Job summary

Path 2 Life Care Pty is seeking a motivated Support Worker to provide group-based and 1:1 in-home/community supports. You’ll facilitate activities, encourage participation, and tailor support to each participant’s goals in a positive, inclusive environment.

You’ll demonstrate initiative, leadership, and strong communication to help participants build independence and social connections within group and community settings.

Qualifications

• CPR/First Aid and Certificate III in Individual Support or equivalent.
• Legal right to work in Australia.
• Fully vaccinated against COVID-19.
• Valid driver’s license and access to a car.
• NDIS/disability support experience preferred.
• Strong English communication skills.
• Police Check and NDIS Worker Screening preferred.

Responsibilities

• Support participants in group activities and social programs, encourage participation.
• Provide 1:1 community and in-home supports tailored to goals.
• Facilitate recreational, social, and skill-building activities promoting independence.
• Assist with community access, outings, appointments, and social activities.
• Support light domestic tasks and occasional personal care.

• Support participants with light domestic tasks and occasional personal care when required.

• Encourage positive engagement, inclusion, and respectful communication.

• Maintain professional documentation and communication with the support team.

• Demonstrate initiative and leadership in supporting participants’ goals and daily activities.

Qualifications

• Relevant certifications (e.g., CPR, First Aid, Certificate III in Individual Support or similar).

• Legal right to work in Australia.

• Fully vaccinated against COVID-19.

• Valid driver’s license and access to a car.

• Experience in NDIS or disability support roles preferred.

• Strong English communication skills.

• Police National Check and NDIS Worker Screening preferred.
