Job summary

Care Compass Collective Pty Ltd is a Queensland-based NDIS provider offering psychosocial and mental health focused disability support. Our team works in-home, in the community and at times in our Slacks Creek office, delivering person-centred care across South East Queensland.

The Disability Support Worker role is casual and itinerant, supporting participants to reach goals and engage with daily life. You will adapt to each day’s needs and maintain dignity and respect in all tasks.

Qualifications

• Reliable and consistent presence to build trust with participants.
• Calm and steady demeanor when supporting people in distress.
• Clear written communication for shift notes, reports and handovers.
• Willingness to ask questions and escalate when needed.

Responsibilities

• Deliver person-centred support according to each participant's plan.
• Help participants build daily living skills, community access and social connections.
• Assist with meals, household tasks, shopping, appointments and transportation.
• Provide personal care as outlined in the participant's plan.
• Apply trauma-informed, recovery-oriented and strengths-based practice.
• Follow safeguarding, risk assessments and behaviour support plans as written.
• Document shift notes plainly and report incidents as required.
• Conduct a dynamic risk check at the start of each shift.

Care Compass Collective is a Queensland based NDIS provider delivering psychosocial and mental health focused disability support. Our support is relationship led, psychology informed and shaped around each person's goals, needs and everyday life. We deliver in-home support, community participation, mentoring, employment assistance, psychosocial recovery coaching and short-term respite across South East Queensland.

ABOUT THE ROLE

The Disability Support Worker delivers day to day support that helps participants work towards their goals, build skills and confidence, and take part in their community and everyday life. Our participants live with psychosocial and mental health related disability, so much of the work involves showing up consistently, reading a situation well, and adjusting the support to suit the day the person is having.

The role is casual and itinerant, working primarily at participant locations across South East Queensland, with occasional attendance at our Slacks Creek office.

WHAT YOU WILL DO

Deliver person centred support in line with each participant's support plan, adjusting your approach to their preferences, communication style and capacity on the day

Support participants to build daily living skills, community access and social connection Assist with everyday tasks such as meal preparation, household tasks, shopping, appointments and transport Provide personal care where it forms part of a participant's plan, respecting their dignity, privacy and stated preferences

Apply trauma informed, recovery oriented and strengths based practice Follow documented safeguards, risk assessments and behaviour support plans exactly as written Write shift notes in plain, factual, person first language, and report incidents and safeguarding concerns in line with our procedures

Complete a dynamic risk check at the start of each shift, and stop or leave a shift where it is unsafe to continue

WHAT YOU NEED

Current NDIS Worker Screening Check, or willingness to obtain one

Current Working with Children Check (Blue Card) HLTAID011 Provide

First Aid and HLTAID009 Provide CPR Completion of the NDIS Worker Orientation Module, or willingness to complete it before commencing Current Australian driver's licence and access to a reliable, comprehensively insured and registered vehicle

Sound written communication skills for shift notes, incident reports and handover, and confident use of a smartphone app for rostering and documentation Current right to work in Australia WELL REGARDED

A Certificate III or IV in Individual Support, Disability, Community Services or Mental Health, or equivalent experience Previous experience supporting people with psychosocial or mental health related disability Mental Health

First Aid, or training in trauma informed practice or de-escalation
