Job summary

H1 Healthcare is seeking experienced Disability Support Workers in Colac, VIC for flexible agency placements. You will provide person-centred care, support daily living and independence, and work with the care team to ensure safe, meaningful support for clients in home and community settings.

Role involves following support plans, maintaining documentation, and coordinating with the broader care team. Prior CERTIFICATE III/IV and First Aid/CPR are preferred; drivers with reliable transport are

Qualifications

• Disability support experience and/or Certificate III/IV requirement.
• Current worker screening and police checks required.
• Current First Aid/CPR and role-specific competencies.
• Reliable, respectful and person-centred approach.
• A valid driver licence and reliable transport for travel.
• Confidence to follow support plans and escalate concerns.

Responsibilities

• Provide daily living support, routines and independence.
• Deliver personal care and mobility support where needed.
• Assist with community access, appointments and social participation.
• Support clients in home settings.
• Follow individual support plans and clinical guidance where applicable.
• Maintain clear documentation and handover to the care team.
