Job summary

Monash Care Disability Services – Oakleigh VIC is seeking dedicated disability support workers to assist with daily living activities, meal preparation and medication prompting in line with individual care plans.

You will help participants engage in community access, appointments and social activities, while transporting them as needed and supporting independent living skills. Documentation and effective communication with families and the team are essential.

Responsibilities

• Assistance with Daily Living (ADLs), including showering, dressing, grooming and personal hygiene
• Meal preparation and assistance with eating where required
• Medication prompting and support in accordance with care plans
• Household tasks such as cleaning, laundry and maintaining a safe home environment
• Community access, shopping, appointments, recreational and social activities
• Transporting participants to appointments and community activities
• Supporting participants to develop and maintain independent living skills
• Providing companionship and emotional support
• Following individual support plans, behaviour support strategies and participant preferences
• Completing progress notes, incident reports and other required documentation
• Communicating effectively with participants, families and the broader support team
