Job summary

Matrix Healthcare is seeking compassionate Disability Support Workers to join our team across Melbourne’s west and north. The role emphasizes high English proficiency, understanding of the NDIS environment, and building meaningful client relationships.

Experience in personal care and supporting diverse needs is valued. Key responsibilities include delivering tailored support, assisting with daily living activities, and coordinating with the clinical team to monitor progress and update care

Qualifications

• Disability support experience and understanding of NDIS environment.
• Experience with mental health disabilities in community settings preferred.
• Excellent English communication and rapport-building skills.
• Ability to implement person-centered care and support plans.
• Strong documentation and record-keeping abilities.

Responsibilities

• Provide individualized care and support to clients with mental health disabilities.
• Assist with daily living activities, including personal care, meal prep and household tasks.
• Accompany clients to appointments, social activities and community events.
• Develop and implement personalized support plans with clients and families.
• Monitor progress and provide feedback to the clinical team.
• Advocate for clients’ rights and ensure needs are met.
• Maintain accurate client records and follow policies and procedures.
