Job summary

Leora Healthcare is seeking a casual Disability Support Worker to assist a client with complex care needs in Denistone West. The role involves personal care, high-intensity supports, and promoting dignity and independence.

You will work with family and healthcare professionals, handle medication administration, catheter and bowel care, and drive the client’s van to appointments and activities. Current checks and a valid Australian driver’s licence are required.

Qualifications

• Experience in disability support or related care roles.
• Ability to follow detailed care plans and document progress.
• Current first aid and CPR certification.
• NDIS Worker Screening Check is required.
• Working with Children Check (if applicable)
• Valid Australian Driver Licence (Full or P Plates accepted)

Responsibilities

• Provide person-centred support per client care plan.
• Assist with manual handling and safe transfer techniques.
• Provide medication administration assistance.
• Support bowel and catheter care.
• Assist client with high-care needs and complex medical needs.
• Drive client’s van for appointments and social activities.
• Monitor client health and report changes to the team.
• Maintain accurate documentation and progress notes.
