Job summary

Peckys Disability Services in Blacktown, NSW invites applications for a Disability Support Worker on a fixed-term 6-month contract. The role covers day program support, personal care, and community access, with 50 hours per fortnight.

The successful candidate will join a dedicated team delivering active care, using participant plans, data collection, and person-centered support to enhance participant well-being and participation.

Qualifications

• Direct disability support including personal care and medication management.
• Apply participant plans such as behaviour support and epilepsy management.
• Accurate data collection for community participation outcomes.
• Maintain and monitor a participant's person-centered support plan.

Responsibilities

• Provide direct support to people with a disability including personal care and medication management.
• Apply Participant plans ie; behaviour support, epilepsy management, etc.
• Accurate data collection in all areas of community participation.
• Maintaining and monitoring a participant’s person-centered support plan.
• Support participants in community engagement and skill development.
