Job summary

HomePlace seeks a motivated leader to step into a part-time direct support role with day-to-day oversight in participants' homes. You will coach staff, develop goals, manage health care plans and foster community connections.

With at least two years' disability support experience, you will use SRV principles, mentor the team, coordinate schedules and ensure safety, wellbeing and rights of participants.

Qualifications

• Minimum of two years' experience working with people with intellectual or other disability.
• Certificate IV specialising in Disability Studies and/or Human Services.
• Experience coaching support staff in community settings.
• Experience supporting people with complex health care needs.
• Current NDIS Clearance.
• Current Working With Children Check.
• Senior First Aid / CPR certification.
• Hold a current driver’s licence and access to a vehicle.
• COVID vaccination completed.

Responsibilities

• Contribute to support planning, goal development and monitoring participant outcomes.
• Support the implementation of behaviour support, health care and communication plans.
• Facilitate community connections and opportunities for social participation.
• Support participants with health, personal care, medication and medical appointments as required.
• Mentor and coach team members to deliver high-quality support.
• Support the orientation and onboarding of new team members.
• Facilitate team meetings and reflective practice.
• Oversee day-to-day household operations, including budgets and safety.
• Identify and escalate health, safety and safeguarding concerns.
• Build positive relationships with families and health professionals.
