Senami is seeking a Disability Services Coordinator to supervise day-to-day operations in residential and community settings. You will ensure high-quality service delivery aligned with client NDIS plans, promote mental health, well-being, and independence, and lead a multidisciplinary team.

The role requires strong leadership, clinical understanding, and a holistic approach to care planning and risk management within SA. Valid driver’s license and access to a reliable vehicle are expected.

Qualifications

  • Experience with clinical documentation and client record systems (e.g., Careview).
  • Ability to work independently and as part of a multidisciplinary team.
  • Sound understanding of NDIS legislation, the Mental Health Act (SA).

Responsibilities

  • Provide responsive, individualized client and clinical support in residential and community settings.
  • Conduct mental health and risk assessments and develop person-centered care plans.
  • Deliver psychoeducation and therapeutic community activities promoting health and inclusion.
  • Maintain updated documentation including care plans and reports.
  • Ensure care meets NDIS, Senami, and SA legislative standards.
  • Coordinate internal and external referrals and manage crisis interventions.
  • Supervise and mentor Team Leaders and Support Workers.
  • Conduct training sessions and team supervision with the Registered Nurse.
  • Lead day-to-day residential service operations and act as on-call officer.
  • Liaise with stakeholders and external services for integrated care.
  • Monitor service quality and ensure compliance with standards.

Position Purpose

The Disability Services Coordinator is responsible for supervising and coordinating day-to-day operations within Senami's residential and community-based facilities. This role ensures high-quality service delivery aligned with client NDIS plans and promotes mental health, well-being, and independence for individuals living with disability and comorbidities. The role requires strong leadership, clinical understanding, and a holistic approach to care planning and risk management.

Key Responsibilities
Client & Clinical Support
  • Provide responsive and individualized support to clients in residential and community settings
  • Conduct mental health and risk assessments, welfare checks, and develop person-centered care plans
  • Deliver psychoeducation and therapeutic community activities promoting health and inclusion
  • Maintain updated documentation including care plans, assessments, and reports
  • Ensure all care meets NDIS, Senami, and SA legislative standards
  • Coordinate internal and external referrals and manage crisis and emergency interventions
Team Supervision & Training
  • Supervise and mentor Team Leaders and Support Workers
  • Conduct training sessions, house meetings, and team supervision in collaboration with the Registered Nurse
  • Promote a supportive and high-performing team culture
  • Support professional development and onboarding of new staff and students on placement
Leadership & Coordination
  • Lead day-to-day residential service operations
  • Act as on-call officer as required, managing after-hours service matters
  • Liaise with stakeholders, allied health professionals, and external services to ensure integrated care
  • Monitor service quality and ensure compliance with organizational and legislative standards
Required Qualifications and Experience
  • Tertiary qualification in Health Sciences, Nursing, Social Work, Psychology, or equivalent
  • Minimum 3–5 years' experience in disability and mental health services coordination
  • AHPRA registration (desirable)
  • Current and valid driver's license
  • Access to a reliable vehicle
Mandatory Checks and Certifications
  • National Police Clearance
  • Working with Children Check
  • NDIS Worker Screening Clearance
  • DCSI clearance (if applicable)
  • First Aid, Mental Health First Aid, MAPA/Safety Intervention Training
  • Valid working rights in Australia
  • Excellent written, verbal, and interpersonal communication
  • Strong leadership, time management, and decision-making skills
  • Experience with clinical documentation and client record systems (e.g., Careview)
  • Ability to work independently and as part of a multidisciplinary team
  • Sound understanding of NDIS legislation, the Mental Health Act (SA), and related compliance requirements
