Job summary

Tender Loving Care Disability Services – Bankstown NSW is seeking a Disability/Aged Carer to deliver high‑quality, person‑centred care to participants in the community and home settings.

You will support daily living activities, personal care, mobility and community participation, guided by individual support plans and health plans, while promoting dignity, choice and independence for each participant.

Qualifications

• No experience required for this role.
• Current and valid relevant driver’s licence if required for the position.
• Current First Aid and CPR Certification.
• Current NDIS Worker Screening Check where applicable.
• Ability to meet the inherent requirements of the position including physical and manual handling.

Responsibilities

• Provide respectful, individualised and person-centred support based on each participant's needs and support plan.
• Promote participant choice, dignity, independence and participation in decision‑making.
• Provide personal care and assistance with activities of daily living where required.
• Assist participants with showering, dressing, grooming, toileting and continence support.
• Support participants to access community, recreational, educational, vocational and social activities.
• Accompany participants to appointments, shopping, social activities and community events.
• Follow individual support plans, health plans, risk assessments and behaviour support plans as applicable.
• Communicate effectively with Team Leaders, Support Coordinators, Service Managers, families, carers and other stakeholders.

About the Role

The Disability / Aged Carer is responsible for providing high-quality, person-centred care and support to people living with disability and/or older people, enabling them to live as independently and safely as possible.

The role involves supporting participants with their daily living needs, personal care, community participation, social engagement, household activities, and other supports identified in their individual support plans.

Key Responsibilities

• Provide respectful, individualised and person-centred support based on each participant's needs, preferences, goals and support plan.
• Promote participant choice, control, dignity, independence and participation in decision‑making.
• Provide personal care and assistance with activities of daily living, where required and within the worker's training and scope of duties.
• Assist participants with showering, dressing, grooming, toileting and continence support as required.
• Support participants to access community, recreational, educational, vocational and social activities.
• Accompany participants to appointments, shopping, social activities and community events as required.
• Promote inclusion and participation while respecting each participant's choices and preferences.
• Follow individual support plans, health plans, risk assessments and behaviour support plans as applicable.
• Monitor and report changes in a participant's health, wellbeing, behaviour or support needs.
• Support participants in accordance with their individual behaviour support plans where applicable.
• Use respectful, calm and person-centred approaches when responding to behaviours of concern.
• Maintain participant dignity and safety when responding to challenging situations.
• Communicate effectively with Team Leaders, Support Coordinators, Service Managers, families, carers and other relevant stakeholders.

Requirements

• Previous experience working with people with disability, older people, or other individuals requiring care and support.
• Demonstrated understanding of person-centred care and support.
• Ability to support participants with personal care and activities of daily living.
• Strong communication and interpersonal skills.
• Ability to work respectfully with people from diverse cultural and personal backgrounds.
• Ability to follow individual support plans, procedures and instructions.
• Ability to maintain accurate documentation and progress notes.
• Ability to recognise and report risks, incidents and changes in participant wellbeing.
• Ability to work independently and as part of a multidisciplinary team.
• Good organisational and time-management skills.
• Current and valid relevant State/Territory Driver's Licence, where required for the position.

• Experience supporting people with complex physical, intellectual, psychosocial or behavioural support needs.
• Experience supporting participants with complex health needs.
• Experience working with people with dementia or age-related conditions.
• Knowledge of NDIS Practice Standards and person-centred approaches.
• Knowledge of aged care standards and principles.
• Experience implementing positive behaviour support strategies.
• Current medication administration training, where relevant to the role.
• Manual handling training.
• Experience working in SIL, group home, respite, day program or in‑home support environments.
