Job summary

Hillsberry Care Pty Ltd is seeking compassionate Disability and Aged Carers to join our team in Sydney. You will provide person-centred care to people with disabilities and older Australians in their homes and within the community, helping them maintain independence, dignity, safety and quality of life.

You will support clients with daily living activities, encourage social participation, and follow individual care plans while respecting preferences and goals.

Qualifications

• Certificate III in Individual Support (Ageing and Disability) required or equivalent.
• Current First Aid and CPR certification.
• NDIS Worker Screening Check and other required clearances.
• Valid driver’s licence and reliable transport, if required.
• Strong communication skills and ability to work in a team.

Responsibilities

• Provide personal care including showering, dressing, grooming and toileting.
• Assist with daily living activities, meal prep, feeding and light domestic duties.
• Support mobility, transfers, and use of mobility aids safely.
• Accompany clients to medical appointments and community activities.
• Follow care plans and safeguarding strategies; observe and report changes.

Job description

We are seeking compassionate, reliable, and dedicated Disability and Aged Carer to join our team. The successful candidates will provide person-centred care and support to people with disabilities and older Australians, helping them maintain their independence, dignity, safety, and quality of life.

You will support clients in their homes and within the community, assisting with daily living activities and encouraging social participation while respecting individual needs, preferences, and goals.

Key Responsibilities

• Provide personal care, including showering, dressing, grooming, and toileting assistance.
• Assist with daily living activities, meal preparation, feeding, and light domestic duties.
• Support clients with mobility, transfers, and the safe use of mobility aids.
• Assist clients to attend medical appointments, social outings, and community activities.
• Support older clients and people with disabilities to maintain independence and wellbeing.
• Follow individual care plans, support plans, and relevant behaviour support strategies.
• Observe and report changes in clients' physical health, emotional wellbeing, or behaviour.
• Maintain accurate progress notes, incident reports, and other relevant documentation.
• Follow infection prevention, workplace health and safety, privacy, and manual handling procedures.
• Communicate professionally with clients, families, colleagues, and healthcare professionals.
• Promote clients' dignity, choice, inclusion, and respect at all times.

Skill & Experience

• Certificate III in Individual Support (Ageing and Disability), or an equivalent relevant qualification where required.
• Previous experience in disability support, aged care, home care, or community services is desirable.
• Current First Aid and CPR certification, as required by the role.
• Appropriate NDIS Worker Screening Check and other required clearances for the services provided.
• Valid driver's licence and reliable transport, if required.
• Good written and verbal communication skills.
• Ability to work independently and as part of a team.
• Reliable, flexible, patient, and compassionate approach to care.
