Bayside Health is seeking a Digital Navigator to work within the Interconnected Care Program, expanding digitally enabled care across hospital and community settings.

You will help patients, carers and clinicians engage with digital health tools, including apps, portals and remote monitoring devices, focusing on onboarding, technical support and administrative coordination.

The Department

We are seeking a motivatedDigital Navigatortowork as part of our Interconnected Care Programto expand our care outside of hospital throughdigitallyenabled care.

The Role

You will help patients, carers, and clinicians engage with digital health tools, including apps, portals, and remote monitoringdevices. The role focuses on patient onboarding, technical support, and administrative coordination to ensure safeandefficient,digitally enabled care.

Key Responsibilities

• Onboard and support patients using digital platforms and devices
• Provide first-line technical support to patients and staff
• Support patients with appointments, communications, and records
• Maintain accurate documentation and patient lists
• Support administrative tasks and workflow improvements

About You

• Certificate III/IV in Business/Medical Administration (or equivalent)
• Strong digital skills and confidence supporting others with technology
• Excellent communication and organisational skills
• Experience supporting diverse patients, including those with low digital literacy
• Professional, adaptable, and committed to patient-centred care
