Job summary

Healthcare Australia (HCA) is expanding its Homecare My Way division in Melbourne and seeks a Customer Care Manager to oversee self-managed client relationships in a virtual, office-based role from Melbourne CBD. You will manage client care initiatives, ensure compliance and help guide clients through arranging services to maintain independence at home.

The successful candidate will bring strong Aged Care or health services knowledge, standout customer service experience, excellent time

Qualifications

• Strong understanding of Aged Care and/or health and community services.
• Experience in customer-focused environments.
• Excellent time management and prioritisation skills.
• Exceptional written and verbal communication abilities.
• Willingness to learn and adapt in a fast-paced environment.
• Nursing background is highly desirable.

Responsibilities

• Develop care plans that support delivery of quality outcomes for clients in line with funding.
• Actively promote Homecare My Way services to meet growth targets.
• Complete all funding and debt management activities.
• Ensure all client documentation and case notes are up to date, complete and of a high standard.
• Support business development and onboarding, including assessment and suitability to provide services per policies.

Customer Care Manager - Home Care (Self-Managed Clients)

Based inMelbourne CBD, the Care Manager manages the client relationship through the delivery of excellent customer service and the implementation of client care initiatives. Managing a caseload of self-managed clients in a virtual capacity - no home visits required, you will help guide our older clients through the process of arranging their own services to help them maintain their independence at home. You would be responsible for ensuring that all service delivery and compliance reporting requirements are met.

Day to day you will:

• Develop care plans that support the delivery of quality outcomes for clients in line with their funding
• Actively promote Homecare My Way services to meet growth targets
• Complete all funding and debt management activities
• Ensure all client documentation and case notes are up to date, complete and of a high standard
• Support the business development and onboarding function, including the assessment and suitability to provide services in line with Homecare My Ways’ related policies and procedures

A successful candidate will demonstrate a strong understanding of Aged Care and/or other health and community services. You will bring experience fromworking in customer focused environments and have excellent time management and prioritisation skills, exceptionalcommunication skills, both written and verbal - imperative to working in this 'virtual' office-based role. You will be able to demonstrate a willingness to learn and adapt to changing environments/ markets. Candidates with a background in nursing are highly desirable.
