Job summary

NSW Health's Justice Health and Forensic Mental Health Network in Broken Hill is seeking an Aboriginal Health Practitioner/Worker to join the Primary Care team. You will deliver flexible, culturally safe health care to Aboriginal and/or Torres Strait Islander people, focusing on prevention and management of chronic conditions within the custodial context.

• Aboriginal Health Practitioner: Certificate IV in Aboriginal Primary Health Care (Practice) and AHPRA registration.
• Aboriginal Health Worker: Certificate III in Aboriginal Primary Health Care; pathway to Cert IV with support.

Responsibilities

• Provide flexible, holistic, culturally sensitive health care to Aboriginal and/or Torres Strait Islander people.
• Collaborate with multidisciplinary teams to deliver timely, patient-centered care.
