Tradewind Australia is seeking an experienced Housing & Homelessness Case Manager to support a Crisis Accommodation Program (CAP) in Gladstone. This role provides client-centred case management from intake through to exit, with a mix of face-to-face and online support to help clients navigate crisis accommodation toward safe, sustainable housing outcomes.

Qualifikationen

• Minimum Certificate III in Community Services (or equivalent) is required.
• Current Driver's Licence is required.
• Police Check and Blue Card are required.
• Experience in housing/homelessness or related community services is preferred.
• Ability to work independently and as part of a team.

Aufgaben

• Provide ongoing case management to clients experiencing homelessness or housing instability.
• Attend the accommodation property for face-to-face client engagement.
• Maintain regular telephone contact with clients to monitor wellbeing and progress on case plans.
• Complete intake, assessment and case planning from entry through to exit.
• Assist clients to identify and address barriers to securing longer-term accommodation.
• Provide practical support, referrals and advocacy to relevant services.
• Develop and maintain relationships with clients and service providers.
