Job summary

Claro Disability Services seeks a dedicated Support Worker to join our Croydon Park team, delivering person-centered supports to clients with complex needs across mobility, medical care, daily routines and community access.

You’ll tailor supports to each client’s goals, collaborate with families and healthcare professionals, and help promote independence and meaningful participation through flexible, shift-based rosters.

Qualifications

• Experience or interest in complex care, disability, and health nuances.
• Strong communication skills and ability to read cues.
• Reliability and professionalism, with good notetaking and teamwork.
• Flexible and adaptable to changing shifts and needs.
• Ability to respect client autonomy and advocate for preferences.
• Full Australian Driver's Licence required for client transportation.

Responsibilities

• Person-centered planning: discuss goals and routines with client and family.
• Complex physical care: assist with mobility, transfers, positioning, daily tasks and devices.
• Medical supports: monitor changes, follow plans, liaise with care team.
• Social time and participation: facilitate meaningful activities and community access.
• Autonomy and advocacy: support decision-making and independence.
• Cultural humility: respect diverse backgrounds, beliefs and ways of being.
