Job summary

Claro Disability Services is seeking a compassionate care worker to support clients with complex needs in a flexible shift role. You will work closely with families, clinicians, and other support workers to implement person-centered plans and ensure safe mobility, daily tasks, and meaningful social activities.

Ideal candidates are warm, patient, and collaborative, with a focus on independence and dignity for clients, and are comfortable adjusting to changing needs and shifts.

Responsibilities

• Provide person-centered planning and support to clients as required
• Assist with mobility, transfers, daily tasks, and devices per care plan
• Monitor client well-being and liaise with the care team for changes
• Support social participation and community access aligned with client interests
• Respect consent, promote independence, and advocate for clients
