You will tailor supports to individual goals, collaborate with families and professionals, and adapt to shifting schedules while building meaningful relationships with clients.

Qualifications

• Must be person-centered, warm and caring.
• Interest or experience in complex care, disability and health.
• Excellent communication; listens, reads cues and adapts.
• Reliable and professional; good notetaking and teamwork.
• Flexible and adaptable; comfortable with changing needs and shifts.
• Values fit: Energy, Nurture and Care, Integrity, Listen and Connect.
• Nice to have: manual handling, epilepsy management, or disability-support training.

Responsibilities

• Provide person-centered planning with client and family.
• Support complex physical care: mobility, transfers, and daily tasks.
• Coordinate medical supports and liaise with care team.
• Support social participation and community access.
• Promote autonomy and advocacy; respect consent.
• Demonstrate cultural humility in all interactions.
