Job summary

Claro Disability Services is seeking a compassionate support role to work with clients with complex needs in Victoria. You’ll coordinate care plans, assist with mobility and daily tasks, and liaise with medical teams to monitor changes in conditions.

You will enable meaningful social participation, support autonomy, respect cultural preferences, and collaborate with families and health professionals to deliver high-quality, person-centered care.

Qualifications

• Person-centered, warm, patient and caring approach.
• Great communicator with strong listening and observation skills.
• Reliable, professional and able to work in a team.
• Flexible and adaptable to changing shifts and client needs.
• Desirable but optional: manual handling, epilepsy management or disability-support training.

Responsibilities

• Engage in person-centered planning with client, family and advocates.
• Provide complex physical care including mobility, transfers and positioning.
• Follow medical plans and liaise with the care team for changes.
• Support social participation and community access aligned with interests.
• Promote autonomy, consent and independence while respecting culture.
• Respect and respond to diverse backgrounds and beliefs.
