Qualifications

• Reliable, professional with good notetaking and teamwork.

Responsibilities

• Person-centered planning with the client and family/advocates.
• Complex physical care including mobility, transfers, and devices.
• Medical supports: monitor changes, follow plans, liaise with the care team.
• Social time and participation aligned with interests.
• Autonomy and advocacy: support decision-making and consent.
• Cultural humility: respect backgrounds, beliefs and ways of being.
