Job summary

Right at Home is seeking a Personal Care Worker to support clients in their own homes under a client care plan. You will perform personal care, assist with daily living activities, nutrition and mobility, while maintaining confidentiality and safety.

Responsibilities include documenting services, reporting changes to the Care Manager, and collaborating with the care team to ensure holistic, client-focused care in a home setting.

Qualifications

• Hold a valid CPR and First Aid certification.
• Certificate(s) in Aged Care and/or Home and Community Care are required.
• Experience or study related to home care is preferred.

Responsibilities

• Assist clients with activities of daily living per care plan.
• Provide personal care including hygiene, mobility, nutrition support.
• Document services rendered using CRM systems or client records.
• Communicate client needs to Clinical Care Manager promptly.
• Respect client values, rights and confidentiality at all times.
• Collaborate with care team to deliver holistic, client-focused care.
