Job summary

Pearl Home Care Gladstone is seeking a Qualified Support Worker to provide high-quality, person-centred support to consumers in their homes and the community. You will work under the Care Manager and Resource Coordinator to deliver services in line with individualised care plans.

Applicants should have Certificate III in Individual Support (Ageing and/or Disability) or equivalent, a National Police Check, First Aid, CPR, Manual Handling, and a current Working with Children Check where required,

Qualifications

• Successful completion of Certificate III in Individual Support (Ageing and/or Disability) or equivalent.
• A National Police Check meeting requirements.
• First Aid Certificate and CPR certification.
• Current Working with Children Check where required.

Responsibilities

• Read and understand care plans and deliver services accordingly.
• Provide personal care and daily living assistance.
• Report changes in health, wellbeing or condition to the Care Manager.
• Maintain privacy, dignity and confidentiality.
• Complete documentation including progress notes within deadlines.
• Safely transport and accompany clients to appointments and activities.
• Prepare meals per dietary requirements and instructions.
• Encourage independence and involvement in decisions about care.
