Pro Care Assist Support Services Pty Ltd seeks a dedicated Support Worker to provide person-centred in-home support to older people and individuals living with disability. You will assist with daily living tasks, personal care, and community participation according to each client’s support plan.

The role emphasizes dignity, respect for cultural preferences, confidentiality, and teamwork, with responsibilities including safety checks, record-keeping, and accompanying clients to appointments.

Formación

• Must have the legal right to work in Australia.
• Hold a valid NDIS Worker Screening Check where required.
• Hold a valid Working with Children Check where required for the role.
• Hold or obtain a National Police Check where required.
• Complete required NDIS Worker Orientation and other mandatory training.
• Hold a current First Aid and CPR certificate, where required.
• Hold a current Australian driver’s licence where driving is an inherent requirement of the position.
• Demonstrate compassion, empathy and respectful person-centred communication.
• Demonstrate reliability, punctuality, patience and professionalism.
• Ability to maintain confidentiality and work independently and as part of a team.

Responsabilidades

• Assisting with personal hygiene, showering, dressing and grooming.
• Assisting with toileting and continence care.
• Supporting mobility, transfers and positioning.
• Assisting with eating and meal preparation where required.
• Supporting medication-related tasks within training and procedures.
• Assisting with basic household tasks such as cleaning and laundry.
• Preparing meals and assisting with grocery shopping as per the plan.
• Helping clients access the community and participate in activities.
• Accompanying clients to appointments, shopping and social events.
• Maintaining accurate records including progress and shift notes.
