Job summary

Pearl Home Care Pty in the Sunshine Coast is seeking a dedicated Participant Care Partner to deliver participant-centered care, manage care plans and coordinate services within the community. You will support mobility, social participation and goal-driven care while adhering to privacy and safety.

A 2-year case management background in aged care, a valid police check, and an Australian driver's license are essential.

Qualifications

• 2 years experience in case management within aged care services.
• Australian Driver's License.
• Valid police check from within the last 3 months.
• Clinical experience within Aged Care is welcome but not essential.

Responsibilities

• Enablement & Re-ablement: support participants to regain skills and embed therapy recommendations into daily routines.
• Social, emotional and community participation: provide companionship and encourage engagement in activities.
• Observation, documentation & reporting: monitor changes and document in systems, report risks or deterioration.
• Support goals and care plans: work with participants to understand priorities and follow care plans.
