Claro Disability Services in Hoppers Crossing, VIC, is seeking a compassionate Support Worker to assist clients in the community with mobility, daily tasks, and meaningful activities. The role requires building trust, coordinating with families and medical teams, and tailoring supports to individual goals.

Shifts are flexible, with opportunities to form genuine relationships and make a real impact in clients’ daily lives.

Qualifications

• Warm, patient and genuinely caring; able to build trust with clients.
• Interest or experience in disabilities and health nuances.
• Strong communication skills and ability to adapt to different styles.
• Reliable, with good note-taking and teamwork.
• Flexible with changing needs and shifts.

• Optional training in manual handling or epilepsy management welcome.

Responsabilités

• Person-centered planning: chat with client and family to understand goals and routines.
• Mobility and daily task support: assist with transfers, positioning, and daily activities as outlined.
• Medical supports: monitor changes, follow care plans, liaise with the care team.
• Social time and community access: enable meaningful activities aligned with interests.
• Autonomy and advocacy: support decision-making and respect consent.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

• Optional but nice: Manual handling, epilepsy management, or disability-support training, experience with para quad highly regarded
