Claro Disability Services in Australia provides person-centred supports to people with complex needs, helping them live independently with dignity. This role involves working closely with clients, families and care teams to tailor goals, plan routines, and facilitate mobility, daily tasks and meaningful activities.

You will contribute to a respectful, inclusive culture, build trusting relationships, and adapt to changing needs and shifts, delivering compassionate, high-quality care every day.

Qualifications

• Warm, person-centered approach to care.
• Excellent communication with clients, families and teams.
• Reliable, flexible and professional with good note-taking and teamwork.
• Interest or experience in complex care, disability and health nuances.
• Optional but nice: manual handling or epilepsy experience.

Responsabilités

• Conduct person-centered planning with client and family to understand goals and routines.
• Provide complex physical care: mobility, transfers, positioning, daily tasks and devices.
• Monitor medical needs, follow plans and liaise with care team.
• Support social time, participation and community access aligned with interests.
• Promote autonomy, advocate, respect consent and independence.
• Respect cultural backgrounds and values in all interactions.
