Job summary

Responsibilities

• Person-centered planning with client and family/advocates.
• Assist with mobility, transfers, positioning, daily tasks, and devices as per plan.
• Monitor changes in health and liaise with the care team as needed.
• Provide social time and community participation aligned with client interests.
• Support autonomy and advocacy and informed decision making.
