Job summary

Responsibilities

• Chat with client and family/advocates to understand goals and routines.
• Assist with mobility, transfers, daily tasks, and devices per plan.
• Monitor changes, follow care plans, and liaise with the care team.
• Facilitate social activities and community access aligned with interests.
• Support decision-making and promote independence where possible.
• Respect cultural backgrounds and beliefs in care.
