Job summary

Albury Wodonga Health (AWH) invites applications for a Palliative Care Social Worker in the Community Palliative Care Team. You will undertake psychosocial assessments, counselling, advocacy, care coordination and supportive interventions for patients, carers and families on palliative care services in community and outpatient settings.

This permanent part-time role (32 hours per week) sits within a multidisciplinary team committed to compassionate, person-centred care, professional development

Qualifications

• Bachelor of Social Work with eligibility for AASW.
• Experience in healthcare, community health or palliative care.
• Experience psychosocial assessments, counselling, advocacy and case management.
• Excellent communication and interpersonal skills.
• Ability to work in a multidisciplinary team.
• Strong organisational and time management skills.
• Commitment to high-quality, person-centred care.
• Current Victorian or NSW Driver Licence.

Responsibilities

• Undertake comprehensive psychosocial assessments, counselling, advocacy and care coordination.
• Provide supportive interventions for patients, carers and families receiving palliative care.
• Work within a multidisciplinary team to deliver compassionate, person-centred care in community and outpatient settings.
