Qualifications

• Bachelor or Masters of Social Work, with a minimum of 3 years experience.
• Registration or eligible for full registration with the Australian Association of Social Work (AASW).
• Demonstrated clinical knowledge and clinical expertise in social work relevant to hospital settings.
• Understanding and applied knowledge of NDIS.
• Proven history of ability to successfully relate and communicate with all levels of internal and external stakeholders.
• Experience in utilising data collection processes and demonstrated skills in data analytics.

Responsibilities

• Deliver high-quality social work services as part of the rehabilitation team.
• Provide psychosocial assessment, therapeutic intervention, advocacy, and discharge planning.
• Support patients and their families to adjust to illness, injury, disability, and changes in functioning.
• Collaborate with interdisciplinary team to develop safe, effective, person-centred care pathways.
• Participation in weekend shifts and the on-call roster is required.
