Albury Wodonga Health (AWH) is seeking a Palliative Care Nurse Practitioner to provide clinical leadership and specialist palliative care consultations within the Wodonga Community Palliative Care service. The role is part-time (24 hours per week) in a multidisciplinary team focused on holistic patient and family care in the community.

• Current registration & endorsement with AHPRA as a Nurse Practitioner of Palliative Care.
• Demonstrated ability to work autonomously & collaboratively in an advanced & extended clinical role.
• Demonstrated capability to communicate effectively with a range of divergent health care professionals.
• Demonstrated ability to engage with & promote reflective practiced across the nursing workforce.
• Demonstrated ability to apply & promote evidence informed practice at point of care.

The Palliative Care Nurse Practitioner will be a valued member of the Wodonga Community Palliative Care service to provide clinical leadership and mentoring and undertake specialist palliative care clinical consultation to enhance the quality of life for a person with a life limiting illness. As a permanent part time role, the Palliative Care Nurse Practitioner will work as a member of a multidisciplinary team which is inclusive of Clinical Nurse Specialists, Occupational Therapist, Social Worker, Grief and Bereavement worker and Volunteer Coordinator, that encompasses support of holistic needs to alleviate suffering of patients and their families in the community setting.
