Job summary

Albury Wodonga Health (AWH) Social Worker role involves delivering psychosocial assessments, therapeutic interventions, advocacy and discharge planning within a multidisciplinary rehabilitation team.

The position requires 3+ years of social work experience, AASW registration or eligibility, and a current driver’s license; weekend shifts and on-call duties are expected. Albury/Wodonga region offers growth and quality of life benefits.

Qualifications

• Bachelor or Masters of Social Work with at least 3 years experience.
• AASW registration or eligibility for full registration.
• Demonstrated clinical knowledge in hospital social work.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families.
• Provide therapeutic intervention and advocacy.
• Assist with discharge planning and care pathway development.
• Support patients and families to adjust to illness, injury and disability.
• Collaborate with the interdisciplinary team to optimise rehabilitation outcomes.
• Participation in weekend shifts and on-call roster.
