Job summary

Wesley Mission Queensland Limited (WMQ) is seeking a CHSP Care Partner based in Chermside to support older people to live independently, safely, and with dignity in their own homes. You’ll work with customers, families, and care teams to deliver responsive, high‑quality community aged care services.

Qualifications

• 3–5 years’ experience in aged care or community services.
• Certificate III in Aged Care, Community Services, or related field.
• Experience in customer engagement, service coordination, and working within regulated funding models (e.g. CHSP).
• Strong understanding of Consumer Directed Care, wellness, and reablement principles.

Responsibilities

• Act as primary contact for customers and supporters, delivering person‑centred customer service.
• Develop, review, and coordinate individualised care plans focused on wellness and independence.
• Maintain regular and meaningful communication with customers, families, and stakeholders to ensure services remain aligned with needs and goals.
• Identify changes in customer circumstances and respond quickly, adjusting services and facilitating referrals where required.
• Work closely with clinical and allied health teams to ensure coordinated and holistic care.
• Coordinate services, equipment requests, and procurement to ensure continuity of care.
• Monitor, record, and respond to incidents, risks, and customer feedback in a timely and professional manner.
• Maintain accurate documentation, data entry, and reporting in line with legislative, funding, and WMQ requirements.
• Participate in care team meetings, case conferences, and continuous quality improvement activities.
• Promote WMQ services and build strong networks to support business growth and positive customer outcomes.
