Collarcare Group is seeking passionate Personal Care Workers to support residents in aged care facilities across Melbourne VIC. You will assist with daily living, document conditions, and help with medications in a compassionate, patient-centered environment.

You will work closely with registered nurses to maintain a holistic care approach, support nutrition and hydration, and ensure adherence to aged care standards. A positive, empathetic attitude and strong teamwork are essential.

Qualifications

• Certificate III in Individual Support or equivalent required.
• COVID vaccination including booster.
• Minimum 3 months experience in Australian aged care facility.
• Current Police Check and NDIS Clearance or willingness to obtain.
• Valid work rights.
• Current CPR & First Aid.
• NDIS Worker Screening or willingness to obtain.

Responsabilités

• Assist residents with daily living including personal care and grooming.
• Document resident’s condition and assist with medications.
• Support meals and monitor nutrition.
• Infection control and prevention.
• Assist the registered nurse to follow holistic care model at all times.
• Assist in admission and easing transition for residents and families.
• Liaise with residents, family, doctors and allied health.
• Act in accordance with aged care standards on every shift.

What do you need to have?

• Certificate III Individual Support or Equivalent

• 3 Months minimum working within an Australian Aged Care Facility
• Current Police Check and NDIS Clearance or willingness to obtain these.
• Valid work Rights.
• Current CPR & First Aid
• NDIS Worker Screening or willingness to obtain

What will you be doing?

• Assisting residents with all aspects of daily living including personal care, grooming and supporting emotional needs.
• Documenting resident’s condition and assisting with the administration of medications.
• Assisting with meal consumption and monitoring nutrition.
• Infection control and prevention.
• Assisting the registered nurse to ensure a holistic model of care is followed at all times.
• Assisting in admission and easing the transition for residents and their families.
• Liaison with all stakeholders including residents, family, doctors and allied health.
• Acting in accordance with aged care standards on every shift.
