Job summary

Nirayu Disability Services Pty Ltd is seeking a Community Worker to provide essential support to clients in their homes and in the community. The role focuses on assisting with daily living activities, promoting independence, and facilitating social inclusion while maintaining a safe, respectful care environment.

The ideal candidate holds a Diploma or higher in a related field with 1–2 years of experience, with potential for substitution if needed. Shortlisted applicants will be contacted.

Qualifications

• Diploma or higher in a related field with 1–2 years of work experience.
• Three years of relevant experience may substitute for the formal qualification.

Responsibilities

• Deliver individualised support in accordance with client care plans.
• Assist clients with personal care and everyday living activities.
• Encourage independence, community participation, and social inclusion.
• Observe client wellbeing and promptly report significant changes.
• Maintain accurate client records and service documentation.
• Build respectful relationships with clients, families, and support professionals.
• Comply with organisational policies, workplace health and safety requirements and applicable care standards.
• Support the delivery of reliable, compassionate, and high-quality community services.
• Contribute positively to the organisation's service objectives and operational requirements.
• Perform other duties reasonably associated with the position.
